State releases annual hospital infections report

CONCORD,ย  NHย โ€“ Theย  New Hampshire Department of Health and Human Servicesย  Divisionย  ofย  Publicย  Health Services has released the 2014ย Healthcare-Associated Infections ย Hospitalย  Reportย  andย  theย  2014ย Healthcare-Associatedย  Infections Ambulatoryย  Surgeryย  Centerย Report based on data from the 35 hospitals and 27 ASC licensed in the Stateย during 2014.

The reports, as mandated by law, report data on the occurrenceย ofย  ย specific ย Healthcare-Associated Infections.

Forย  hospitals,ย  centralย  line-associatedย  bloodstreamย infections, catheter-associated urinary tract infections, and surgical siteย infections following certain heart, colon, abdominal hysterectomy, and kneeย surgicalย  proceduresย  areย  reported.ย  Forย  ASC,ย  surgicalย  siteย  infectionsย followingย  certainย  breast, hernia, and open reduction of fracture surgicalย procedures are reported.

Theย  reports also provide data on hospital and ASC compliance with measuresย thatย  helpย  protectย  patientsย  from healthcare-associated infections. Theseย dataย  includeย  adherenceย  toย  infection prevention practices during centralย lineย  insertions in hospitals, the appropriate use of antimicrobials duringย surgicalย  proceduresย  inย  ASC,ย  andย  influenzaย  vaccinationย  coverage amongย hospital and ASC healthcare personnel.

โ€œSinceย  weย  now have multiple years of data on HAI, trends are beginning toย emergeย  thatย  validateย  andย  enhanceย  efforts to decrease the occurrence ofย theseย  infections,โ€ย  saidย  Dr. Benjamin Chan, State Epidemiologist at DHHS.ย โ€œOverall,ย  our hospitals are doing better than the national average and ourย ambulatoryย  surgical centers are on par with the rest of the country. Whileย thisย  isย  reassuring,ย  our goal is to continually improve and these reportsย will help with that.โ€

Overview: Manchester Hospitals fared as well or better than predicted

Overall,ย  thereย  wereย  fewerย  observedย  infectionsย  thanย  predictedย  in New Hampshireย  hospitalsย  comparedย  with national data. A total of 219 HAI wereย reportedย  by hospitals in 2014, compared with 183 in 2013, 198 in 2012, 110ย inย  2011,ย  114ย  inย  2010,ย  andย  134ย  inย  2009.ย  Thereย  wasย  a statisticallyย significantย  increaseย  forย  overallย  HAIย  betweenย  theย  firstย  year of dataย presented in this report, 2010, and the most recent reporting period, 2014.

However,ย  thisย  pattern was not seen between each individual year from 2010
toย  2014.ย  Many factors may have contributed to this pattern, including theย additionย  ofย  newย  and expanded reporting requirements in 2012. The overallย observedย  numberย  ofย  HAIย  in hospitals in New Hampshire was 15 percent lower thanย predictedย  basedย  onย  nationalย  data;ย  thereย  wereย  alsoย  39 percent fewer centralย line-associatedย  ย bloodstream infectionsย  andย  22 percent ย fewerย  surgicalย  siteย infectionsย  than predicted. There were 26 percent more catheter-associated urinaryย tractย  infectionsย  than predicted, but this difference is not statisticallyย significantย  ย andย  ย theย  ย numberย  ย ofย  infectionsย  observedย  isย  consideredย statistically similar to national data.

Statewideย  hospital adherence to four infection prevention practices duringย central line insertions was 98.3 percent a slight but insignificant decrease fromย 2013ย  (98.4 percent).ย  Theย  overallย  healthcareย  personnelย  influenzaย  vaccinationย coverageย  inย  hospitalsย  wasย  93.5 percent, which has been significantly improvingย since 2008 and exceeds the Healthy People 2020 goal of 90 percent.

Ambulatory Surgery Centers:

Overall,ย  statewideย  infectionย  ratesย  inย  ASC are similar in comparison toย nationalย  data.ย  Aย  total of four surgical site infections were reported byย ASCย  forย  2014,ย  comparedย  withย  six in 2013 and four in 2012.ย  The overallย observedย  numberย  ofย  surgical site infections in New Hampshire ASC was 64 percentย fewerย  thanย  predictedย  based on national data; however, this difference isย notย  significantย  and is considered statistically similar to national data.

Statewide,ย  ASCย  adherenceย  toย  intravenousย  antibioticย  prophylaxis timingย guidelinesย  to prevent surgical site infection was 98.4 percent (similar to 2013),ย andย  theย  overall healthcare personnel influenza vaccination rate was 85.7 percentย (aย  slight but insignificant decrease from last year, during which the rateย was 88.7 percent).

Theย  collectionย  of the data is intended to be a tool for hospitals and ASCย toย  track their progress and identify areas that require special attention,ย forย  DHHSย  toย  seeย  whereย  improvementsย  canย  beย  made,ย  and to help informย consumers.
